Add index macro descriptions to reftex-index-macros-style.

The format of ENTRY-LIST is exactly like reftex-index-macros. See there for details. This function makes it possible to support RefTeX from AUCTeX style files. The entries in ENTRY-LIST will be processed after the user settings in reftex-index-entries, and before the defaults. Any changes made to reftex-index-macros-style will raise a flag to the effect that the label information is recompiled on next use.
